This device highlights] the transition to mobile devices in the medical realm and the benefits for both clinicians and patients. For clinicians, mobile devices and apps may serve as diagnostic aids, med calculators, and clinical references. They can also be used for medical education purposes and access of electronic medical records. For patients, mobile devices and apps are often utilized to find resources and information about diseases, to help with disease management, and to promote lifestyle modification and fitness.2
Today, pulmonary diseases are one of the major causes of mortality in the world. Even though there are different diagnostic tests available such as X-ray and tomography, the stethoscope is still the first, cheapest and the most frequently used diagnostic device for the physicians. In this paper, a smart electronic stethoscope has been designed to help physicians with the diagnosis of the disease using Machine Learning. In order to create a design that satisfies all the needs of the physicians, 15 doctors and medical students from several hospitals have been contacted and interviewed. The developed system has been tested with different machine learning techniques and its efficiency has been shown by obtaining 84% accuracy while classifying respiratory audio.3
The proposed stethoscope can be used in hospitals and telemedicine through an internet of things (IoT) system and a cloud database. The proposed stethoscope includes three parts, namely, a front-end device for electrocardiogram and heart sound measurement, a smart device application (APP), and a cloud server. The ECG-measuring device is designed for single lead measurement and has low power consumption and IoT-based design, which can send real-time electrocardiogram data to the smart device APP. Simultaneously, the heart-sound-measuring device combined with a traditional stethoscope head is used to measure heart sound signals. This device includes an analog front-end circuit and a microphone to filter environmental noises and to record heart sound signals. Heart sound signals are transmitted to the APP by using a Bluetooth Low Energy module. The smart device APP can display synchronized and real-time signals, including ECG and heart sounds. Meanwhile, those signals are recorded in smart devices and are uploaded to the cloud server, where doctors and users can diagnose and monitor healthcare anytime. The cloud server can store previous signals and can realize telemedicine through a web user interface. The proposed intelligent stethoscope is applied on human trials in the National Cheng Kung University Hospital.4

[Eko DUO provides] a software application on smartphones for heart-related physiological signal recording, display, and identification.… The overall measured positive prediction, sensitivity, and error rate of the proposed algorithm are 99.90%, 99.82%, and 0.28%, respectively. During auscultation, doctors may refer to these physiological signals displayed on the smartphone and simultaneously listen to the heart sounds to diagnose the potential heart disease. By taking advantage of signal visualization and keeping the original diagnosis procedure, the uncertainty existing in heart sounds can be eliminated, and the training period to acquire auscultation skills can be reduced.6

How to Use the Eko Duo ECG and Digital Stethoscope
Use the device on various locations and orientations of the chest. Each position will produce a unique heart sound and electrocardiogram tracing.
Capture PCG sounds by placing device anywhere on the body. For best audio, press the device firmly against the skin to ensure good contact.
Capture electrocardiogram by placing and orienting DUO on the chest. The optimal position to capture ECG data is at an inward-facing 45° angle at the upper left pectoral below the clavicle. If the patient has very dry skin, body fat, or hair, conductive gel use may improve the quality of the data.
It enables analysis and acquisition of both PCG and ECG signals in a single pass. We describe the development, experimental evaluation, and comparison of the electrocardiogram signals obtained using our proposed approach and a gold standard medical device, through metrics that allow the evaluation of morphological similarities. Results point to a high correlation between the two evaluated setups, thus supporting the idea of meaningfully collecting electrocardiogram data along medical auscultation points with the proposed form factor. Moreover, this work has led us to conclude that for the studied population, signals acquired on focuses F1, F2, and F3 are usually highly correlated with leads V1 and V2 of the standard electrocardiogram medical recording procedure.7
Doctors and nurses use the stethoscope on patients during a physical assessment in a clinical setting. A licensed medical professional should prescribe the device when used by a patient for in-home monitoring and under the supervision of a clinician. The system provides one source of data that is significant only when used in conjunction with clinician oversight and consideration of other relevant patient data. The device should be used only by qualified clinicians or prescribed to patients with an adequate understanding of the device. Patients that can normally auscultate with an acoustic stethoscope qualify for in-home monitoring.
Follow standard procedures for auscultation, including background noise reduction and optimal patient positioning. The quality of the electrocardiogram is dependent on proper preparation practices, including, but not limited to, patient body temperature and clean contact area. The DUO uses a Bluetooth Class 2 wireless data link. The range of the Bluetooth reduces when objects (walls, furniture, people, etc.) are between DUO and a paired mobile device. To improve Bluetooth connection, reduce the distance and allow a line of sight between the DUO and the mobile device. To transmit sounds and record electrical activity to the Eko App, DUO, and the mobile device must connect via Bluetooth. To fully use certain functions, the mobile device must have an Internet connection. Users of the mobile App and web Dashboard should use networking security features to protect patient data created and stored using this software, in addition to security features embedded in the system.
The Eko Model E5 System use by healthcare professionals is to electronically amplify, filter, and transfer diagnostic sounds and single-channel electrocardiogram waveforms. The Eko Model E5 System also displays electrocardiogram waveforms and phonocardiogram waveforms on the accompanying mobile application for storage and sharing when prescribed or used under the care of a physician. Its use includes recording heart sounds and cardiac murmurs, bruits, respiratory sounds, and abdominal sounds during physical examination in normal patients or those with suspected diseases of the cardiac, vascular, pulmonary, or abdominal organ systems. The device works with adults and pediatrics. The data offered by the Eko device is only significant when used in conjunction with a physician’s knowledge and relevant patient data. Another Eko Stethoscope option is the Eko CORE Stethoscope which is a digital stethoscope but without the electrocardiogram function. Another option if you already have a favoite acoustic stethoscope but want to add digital features from time-t-time is the CORE Stethoscope Attachment. This digital attachment slides in and out of the stethoscope tubing for when it is needed.
The DUO is not for use on infants weighing less than 10kg. Patient health information protection by state, federal, or international/foreign laws regulate the use, storage, transmission, and disclosure of patient. The Eko system employs security features that are compliant with HIPAA policies. Third-party access to patient information without obtaining written authorization from the patient may fall under restricted access. The user is fully responsible for understanding and following all laws that regulate the storage, transmission, and disclosure of any electronic patient data. If the user becomes unable to comply with a law or restriction that applies to the use and disclosure of such data, the user should not proceed to collect or save such information.
This application may require the entry of individually identifiable health information to function. The device stores and recalls records through the use of the patient name, date of birth, and patient ID number. By entering this data, the user assumes all risks and liabilities with using or transmitting such information.
Eko DUO Features
Charge the device by placing the device on the Wireless Touch Charging Pad. Connect the charging pad to the power source using the USB charging cable and power adaptor. The LED light ring will begin to flash, signaling that the device is charging. When fully charged, the light ring will stop flashing.
Note: While charging, it will not connect to the Eko app.
Clean the unit between each patient use. Eko recommends cleaning using 70% isopropyl alcohol wipes or solution. Note: DO NOT immerse the product in any liquid or subject it to any high-pressure/autoclave sterilization processes.
Eko Devices, Inc. expressly warrants that the Eko DUO will materially conform to its published specifications and be reasonably free from defects in materials and workmanship for a period of one year. This period commences on the date of delivery of the device to user and constitutes the "Warranty Period". This Limited Warranty only applies to products used, stored, and handled in the manner recommended by Eko.
