2. How To Enter the VITALITYMEDICAL.COM GIVEAWAY ("Contest") The
Contest begins on the date specified at 12 Midnight Pacific Standard
time and ends in 60 days at 12 Midnight, Mountain Standard Time
("Contest Period"). Mountain Time defines the Contest Period. At least
one $250.00 coupon voucher/credit will be will be given away during the
Contest Period. To enter, first use a computer to access the Entry

3. You can only register for the Contest under one (1) name and one (1)
email address. You cannot use any other person's name or email address
to enter the Contest. You can only enter the Contest using an email
address that you have registered with in your name. You cannot allow
any person other than yourself to enter the Contest using your name or
email address. Once an email address has been registered by an entrant
that email address cannot be used by any other person for purposes of
registration. The email address provided for registration must be valid
and you must be able to receive an email. Entrants will receive our
monthly newsletter including special offers and sales coupons for
valued savings. If entrants desire to be removed from the newsletter
they are automatically removed from the contest giveaway and future
mailings. Failure to follow any part of the official rules can lead to
disqualification from the Contest and eligibility to receive prizes.
4. Eligibility: Open only to legal residents of the United States who
reside (and who are physically located) in the United States. Entrants
must meet age requirements for show and winners are responsible for all
shipping expenses associated with redemption of the prize. Employees of
the Contest Entities (defined below), their immediate families and
members of their same households are ineligible to enter or win.
5. Drawing and Notification: Winners for each Contest Period will be
selected from all eligible entries received during that Contest Period
in a random drawing held approximately the day after the end of the
Contest Period. Potential Winners will be notified by email and the
email shall serve as the prize voucher ("Voucher"). No additional
notification will be sent to winners. If a potential winner cannot be
reached after a reasonable effort has been made during three (3)
business days from the first notification attempt, or if an entrant is
found to be ineligible, or if he/she does not comply with the Official
Rules, the winner will be disqualified.
6. If a potential winner is found to be ineligible (based on the
failure to submit correct information, or cannot or does not comply
with these Official Rules), an alternate winner may be selected.
Sponsor shall not be liable for entries, winner notification responses
or prizes that are delayed, lost, misdirected, misdelivered, illegible,
mutilated, incorrect or incomplete, and such entries will be not be
eligible for this Contest. Once submitted, entries become the sole
property of Sponsor and will not be returned.
7. VitalityMedical.com and any agents or representatives acting on
behalf of the above entities, including, but not limited to, those
involved in the development, production (including prize suppliers),
implementation and distribution of this Contest and their respective
parent companies, affiliates, subsidiaries, service providers and
agencies and their officers, employees and any other person or entity
associated with such entities and/or this Contest or the show ("Contest
Entities") are ineligible to enter or win.
8. Prizes: Odds of winning a prize depend on the number of eligible
entries received. No substitution, transfer, or cash equivalent for the
Voucher is valid, except Sponsor may, in its sole discretion, due to
prize unavailability or for any other reason, make a substitution of
comparable value. The Voucher is: (a) redeemable with a valid driver's
license or other government issued photo identification document; (b)
non-transferable to any party other than the winning entrant; (c) not
accepted by Vitalitymedical.com or any other third party agency; and
(d) not redeemable for cash or credit applied to a credit card. Sale of
the Voucher is strictly prohibited and will render it void. (e) Voucher
must be used for purchase of defined items/categories listed on
Vitality Medicals website. (f) No returns or exchanges permitted under
the contest give away rules.
9. Notice of Winning/List of Potential Winners: A list of potential
winners will be posted on our email newsletter no later than 30 days
following the end of the Contest Period. If a potential winner is found
to be ineligible based on the failure to submit correct information (as
described herein), or cannot or does not comply with these Official
Rules, an alternate winner may be selected for a prize.
10. General Conditions: All taxes (including income taxes) are the sole
responsibility of winners. Return of prize or prize notification as
undeliverable will result in disqualification. Except where prohibited
by law, entry and/or acceptance of prize constitutes permission for the
Sponsor and its agencies to use the entrant's/winner's name, address,
statements, biography, image, and/or likeness for advertising and trade
purposes without further compensation, worldwide and in perpetuity, in
any and all forms of media, now known and hereafter devised, including
without limitation online.
11. Any person attempting to defraud or in any way tamper with the
Contest in the sole discretion of Sponsor, will be ineligible for
prizes and will be prosecuted to the full extent of the law. If, for
any reason, a Contest is not capable of running on the internet as
originally planned, such as tampering, computer malfunction, or
infection by computer virus, Sponsor at its sole discretion, reserves
the right to cancel the Contest. The Contest is void where prohibited
by law. Prizes won by eligible minors will be awarded in the name of
the parent or guardian, who may be required, at Sponsor's discretion,
to complete and return a Certification of Eligibility on minor's behalf.
12. Contest Conditions: This Contest is subject to these Official
Rules. By participating, entrants agree to be bound by these Official
Rules and the decisions of the Sponsor which are final and binding in
all respects.
13. Entrants agree that Contest Entities shall not be liable for losses
or injuries of any kind resulting from acceptance/possession or
use/misuse of prizes, participation in the Contest, or any technical
malfunction of the telephone network, computer online system, computer
dating mechanism, computer equipment, hardware or software, program
malfunctions or technical errors, or any combination thereof, or any
lost, garbled, misdirected, illegible, or delayed data transmissions.
Entrants also agree that the Contest Entities are not responsible or
liable for any injury or damage to an entrant's or third person's
computer and/or its contents, related to or resulting from any part of
the Contest. Contest Entities are not responsible for multiple entries
entered by other than human means, or for entries that are late
(including delayed data transmissions), tampered with, garbled,
incomplete, lost, misdirected, interrupted or otherwise not in
compliance with these Official Rules (and such entries will be
disqualified).
